Now Hiring: Join Central City Concern's Recuperative Care Program's leadership team as a RCP Supervisor (Overnight/Graveyard Shift)

Location: Central City Concern - Evergreen Crossing Primary Care Center (8225 NE Wasco Street, Suite 300, Portland 97220)

Schedule: Monday - Friday (Overnight/Graveyard Shift; 12:00am - 8:00am)

MINIMUM QUALIFICATIONS (REQUIRED)

For formal consideration for the RCP Supervisor position, please ensure your professional/working experience meets the following requirements:

  1. Required: Bachelor's degree in health, business, or public administration, health sciences, public health, or related field.
  2. Required: Two (2) years of progressively responsible experience in medical respite operations, clinical leadership, wider health care or social services operations, or health care business management.
  3. Required: Must possess a current driver's license, pass a DMV background check and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. Must pass an initial drivers training and annual recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.


PREFERRED QUALIFICATIONS

  1. Preferred: Medical Case Management/Care Coordination/Medical Respite/Recuperative Care experience. Those with relevant lived experience are encouraged to apply.
  2. Preferred: Bilingual.

About CCC's Recuperative Care Program

The Recuperative Care Program (RCP) is an innovative service that provides immediate access to emergency housing, primary care, and intensive case management support for individuals who have medical support needs while experiencing homelessness. The service partners with Portland area hospitals and clinics, in concert with CCC's patient-centered medical home, to help clients/patients receive the right level of care and support for their medical and social needs.

About the Role (Job Summary)

The RCP Supervisor works as a member of and leads patient navigator staff providing intensive case management, care coordination, and on site support for medically vulnerable adults facing homelessness upon discharge from the hospital. The team is focused on working with partner hospitals and referring agencies to provide recuperative care for both the acute and ongoing medical needs of patients at Blackburn, and also providing short-term emergency supportive housing, crisis management, referral to appropriate substance abuse, physical health, and mental health treatment, access to benefits and entitlements, appropriate and affordable housing, and further provide solutions to ending homelessness and self-sufficiency through Central City Concern's comprehensive approach.

The RCP Supervisor works in conjunction the RCP leadership team in providing oversight of daily onsite tasks and workflows, participating in collaboration with community partners, and assuring that best practices of intensive case management and trauma-informed complex care coordination are utilized. The RCP Supervisor provides supervision for Patient Navigator staff and directly supports staffing continuity, scheduling, and milieu management, as well as medical, behavioral, and facility incident response activities.

Essential duties and responsibilities:

  1. Work as a member of and support the RCP multi-disciplinary team and operation of program, including enduring adequate and appropriate staffing, ensuring the continuity of client support services, access, referrals, and census management, and providing 24/7 on-call support on a rotating schedule.
  2. Directly supervise and provide coaching to Recuperative Care Patient Navigator staff, engaging in performance improvement plans as appropriate for supervisees.
  3. Provide training to new and existing Recuperative Care staff on program workflows, policies and procedures as needed to maintain program standards.
  4. Provide crisis intervention which may include counseling, supporting clients with de-escalation, performing risk assessments, supporting safety planning, and referring clients to recommended levels of care.
  5. Provide decision-making guidance for Patient Navigator staff at case-specific, program, and community partnership levels.
  6. Monitor attendance and time management of Patient Navigator staff. Mentor team members on effective time management.
  7. Act as a resource for problem solving with Recuperative Care staff.
  8. Support Recuperative Care staff in proceeding through workplace conflict in a constructive manner using effective conflict management processes.
  9. Apply Trauma Informed Care and Harm Reduction principles in providing care for RCP participants and supervision of Patient Navigator staff.
  10. Support team members in the development, implementation and maintenance of case plans for enrolled patients.
  11. Respond to, and support Patient Navigator response to, emergency medical and mental health situations for program participants as needed and appropriate, completing pre-hospital care reports and incident reports when necessary.
  12. Support the program's census and authorization management procedures and protocols.
  13. Collaborate with RCP Leadership Team in maintaining quality assurance systems for all tasks associated with program operation.
  14. Support the coordination of medical, behavioral health and substance use support, and a variety of social services available to patients.
  15. Perform daily and as-needed welfare checks and charting on patients enrolled in the Recuperative Care Program.
  16. Work with Recuperative Care team (including RCP leadership team, Patient Navigators, Logistics Specialist, Housing Specialists, and additional program staff) to ensure quality care for patients, including helping to identify behavioral, psychological, social and medical needs.
  17. Attend and facilitate staff meetings and multidisciplinary case conferences.
  18. Participate in compliance, quality assurance, quality improvement, and program development activities as assigned.
  19. Review hospital charts and medical records and obtain releases from patients to gather collateral information.
  20. Screen and assess patient referrals at Partner Hospitals upon identification of appropriate patients.
  21. Document client encounters and scheduling in Electronic Health Record in a timely manner.
  22. Follow Policies and Procedures of Recuperative Care and Central City Concern Health Services.
  23. Provide transportation for patients in CCC vehicle to and from program related appointments, interviews, and hospitals/clinics.
  24. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  25. Attend all mandatory CCC trainings in a timely manner.
  26. Attend weekly clinical meetings and provide current information to team regarding client's participation in program and other activities.
  27. Perform other duties as assigned.
