POSITION DESCRIPTION

Position: Peer Health Navigator

Supervisor: Health Connections Program Supervisor

Location: Banfield Shelter Motel - 1525 NE 37th Ave

Hours: Monday-Friday 8am-4pm, some nights and weekends as needed

Wage: $22.44 hourly

Eligible for Language Differential

GENERAL RESPONSIBILITIES

Provide peer mentoring, engagement, and navigation to people experiencing homelessness needing assistance to connect to medical, mental health, and substance use disorder programs. This is a non-exempt, represented full-time position.

SPECIFIC RESPONSIBILITIES

  1. Provide peer support and assistance to participants in the Health Connections program to help in connecting with medical, mental health, and addictions programs.
  2. Provide escort for assigned participants utilizing public transportation or medical transport where indicated and provide follow-up support to ensure connection to necessary wellness services.
  3. Provide assistance to assigned participants to understand aftercare instructions, follow up with appointments, and link to on-going care.
  4. Coordinate with community social service partners for access to needed services.
  5. Document service contacts in Service Point and aggregate data for reports.
  6. Other duties as assigned.

PERFORMANCE REQUIREMENTS

Each Transition Projects employee must:

  1. Acquire First Aid/CPR/AED certification within 30 days of hire and be retested biannually thereafter throughout employment.
  2. Complete Bloodborne Pathogens and Narcan Training within 30 days of hire and be retested annually thereafter throughout employment.
  3. Demonstrate reliability by being present for work as scheduled, arriving and leaving on time and taking breaks in expected time frames.
  4. Interact effectively with persons of diverse ethnic backgrounds, religious views, cultural backgrounds, and sexual orientations and treat each individual with respect and dignity.
  5. Attend agency and program staff meetings.

PHYSICAL/MENTAL REQUIREMENTS

The physical and mental demands described here are representative of those that must be met by an employee, with or without accommodation, to successfully perform the essential functions of this job.

  1. Hearing and vision adequate for interaction with participants, interaction with staff and interaction with the general public.
  2. Hearing adequate for telephone work. Vision adequate for close work.
  3. Hand and finger dexterity adequate to operate standard office equipment.
  4. Ability to sit at a desk completing paperwork and working on a computer for lengthy periods.
  5. Ability to walk, bend, stand, sit, reach, stoop, pull, sit, squat, and climb stairs.
  6. Must be physically capable of performing CPR for 15 minutes continuously.

MINIMUM QUALIFICATIONS

The right individual for this position has a passion for helping others, is nonjudgmental, and is able to deal with difficult situations in positive and constructive ways.

  1. Peer Support Specialist Certification or Community Health Worker certification required OR the ability to receive the certification within 6 months of hire.
  2. Understanding of the issues surrounding homelessness, chemical dependency, mental health, domestic violence and sexual assault.
  3. Demonstrated ability to work independently. Ability to appropriately assess a situation and take corrective action as necessary.
  4. Knowledge of services available to low-income and homeless persons through Transition Projects and other agencies or the demonstrated ability to obtain such knowledge.
  5. Demonstrated ability to work as part of a team in the delivery of services to participants.
  6. Computer literacy. Demonstrated ability to use word processing, database, spreadsheet, e-mail, and Internet programs.
  7. Excellent oral, written, and telephone communication skills. Excellent interpersonal skills.
  8. Bilingual English/Spanish fluency is strongly preferred.
  9. Office/clerical experience. Familiarity with office equipment (copier, fax, multi-line phone system, etc.).
